I hereby:

• confirm and agree that I have consented to any Personal Data provided by me to Madd Recruitment and my CV (and all Personal Data contained within my CV) to be used pursuant to the recruitment services provided by Madd Recruitment;
• consent to Madd Recruitment sending this information to:
  • prospective employers and clients of Madd Recruitment in connection with the recruitment process in order for them to assess my job eligibility (Madd Recruitment will not forward the Candidate’s details to any clients without discussing the relevant position with the Candidate first, and obtaining its consent to do so);
  • third parties where Madd Recruitment has retained them to provide services that I or Mad Recruitment’s client has requested. E.g. references, qualifications and criminal reference checking services, verification of the details I have provided from third party source, psychometric evaluation and/or skill tests; and
  • companies and individuals Madd Recruitment engage to perform business functions and services on our behalf. Such functions may include, for example, hosting our web servers, analyzing data, providing legal, accounting and marketing services;
• warrant that all information provided by me is true, accurate and up to date; and
• agree that my consent has been freely given.

This Consent Form is governed by English law and Madd Recruitment and I agree to irrevocably submit to the exclusive jurisdiction of the English courts.

By signing this Consent Form I acknowledge that I have completely read and fully understand the above provisions and agree to be bound by its terms.

Please note that your Personal Data will only be retained for as long as reasonably required by Madd Recruitment and will only be used by Madd Recruitment for the purpose of providing recruitment services. All Personal Data will be processed in accordance with our Privacy Policy.
